PSA birth certificate

The copy printed on PSA security paper, not the one from your city hall.

What it is: Proof of your name, birthday and parents, issued by the Philippine Statistics Authority.
Where to get it: A PSA outlet (walk in), or order online through PSA Serbilis for delivery.
Cost: A fixed PSA fee per copy; delivery costs more. Check psa.gov.ph for the current amount.
How long to allow: About 7 days — ask sooner if you can.

Tips

Check that every letter of your name is right before you leave the counter.
Keep one extra copy — many scholarships ask for it.
